经伤椎椎弓根固定治疗强直性脊柱炎合并胸腰椎骨折

目的探讨经伤椎椎弓根固定治疗强直性脊柱炎合并胸腰椎骨折的疗效,为临床治疗的开展提供依据。方法选择2008年1月～2011年12月本院收治的强直性脊柱炎合并胸腰椎骨折患者24例,将其分为观察组和对照组,每组各12例。观察组给予经伤椎椎弓根固定手术治疗,对照组给予传统手术治疗。观察两组手术前后骨折锥前缘高度、Cobb角,并进行比较。结果术前两组骨折锥前缘高度及Cobb角比较差异无统计学意义（P〉0.05）,两组术后骨折锥前缘高度及Cobb角均较术前明显改善（P〈0.05）,术后12个月,观察组改善情况明显优于对照组（P〈0.05）。结论经伤椎椎弓根固定治疗强直性脊柱炎合并胸腰椎骨折的近期疗效与传统治疗方法相近,但远期疗效及稳定性佳,值得临床推广应用。

Treatment of ankylosing apondylitis combined with thoracolumbar fracture by fixation through the pedicle of fractured vertebra

Objective To discuss the efficacy of fixation through the pedicle of fractured vertebra in the treatment of ankylosing apondylitis combined with thoracolumbar fracture,in order to provide reference for the development of clinical treatment.Methods Twenty-four cases of patients with ankylosing apondylitis combined with thoracolumbar fracture in our hospital from January 2008 to December 2011 were selected and divided into observation group and control group,each group of 12 cases.The observation group was given the therapy of fixation through the pedicle of fractured vertebra operation.The control group was treated with traditional operation.Fractured vertebra height,Cobb angel,internal fixation failure rate and lumbago of the two groups were observed and compared.Results There were no significant differences of the fractured vertebra height and Cobb angel between the two groups before operation(P > 0.05).After operation the fractured vertebra height and Cobb angel were all improved of both groups(P < 0.05),and after 12 months of operation,the improvements of the observation group were better than those of the control group(P < 0.05).Conclusion The recent treatment effect of fixation through the pedicle of fractured vertebra in the treatment of ankylosing apondylitis combined with thoracolumbar fracture is similar with the traditional treatment method,but the long-dated treatment effect and stability are better.It is worthy of clinical application.
