乳腺肿瘤血流动力学与彩色多普勒超声对比研究

目的：探讨乳腺肿瘤血流动力学彩色多普勒超声表现特征。方法：对244名经手术或穿刺活检确诊的乳腺肿瘤患者,共259个肿块进行彩色多普勒多参数检测〔包括肿块血流分级,血流收缩期峰值速度（PSV）,舒张期速度（EDV）,阻力指数Ri〕,对比乳腺浸润性导管癌、导管内乳头状瘤、纤维腺瘤三组间各参数的相关性及差异性。结果：乳腺癌和导管内乳头状瘤的血流显示率高,分别为100%和91%,二者血流信号数目无显著性差异,与纤维腺瘤相比均差异显著（P〈0.001）;乳腺癌PSV高,平均约19cm/s,与导管乳头状瘤及纤维腺瘤比较差异显著（P〈0.01）;三者间EDV无差异性;纤维腺瘤、导管乳头状瘤、浸润性导管癌的RI呈逐级增高趋势,三者间两两比较均差异显著（P〈0.01）。结论：乳腺肿瘤的彩色多普勒参数中RI最具诊断价值。当肿块血管数目多、血流分级为Ⅱ～Ⅲ级,肿块血流速度快PSV〉19cm/s,血流阻力高RI〉0.75时提示乳腺癌可能性大。

Comparative Study of the Breast Tumors Vascularity by Color Doppler Sonography

Objective To evaluate the characteristics of the breast tumors vascularity by color Doppler parameters.Methods 259 lesions of breast tumors of 244 patients were prospectively analysed by color Doppler parameters（PSV、EDV、RI）.Results Hypervascularity were found in invasive ductal carcinoma and mammary intraductal papilloma,There were no statistical differences for the number of tumor vessels between them.PSV were significantly higher in breast cancer group than it in intraductal papilloma and fibroadenoma group（P〈0.01）;There were statistical differences for RI among the breast tumors：cancer groupintraductal papillomafibroadenoma（P〈0.01）.Conclusion RI was the most useful color Doppler parameter for diagnose and differentiate breast tumor.
