19例多发性骨髓瘤临床分析

目的:探讨多发性骨髓瘤(MM)的临床特征。方法:回顾性分析19例MM患者临床资料。结果:①起病隐匿,临床表现多样化,易误诊;②大部分患者表现骨痛和贫血,免疫分型以IgG和IgA最常见;③COP/CTHPOP化疗方案治疗5例初治患者有效率达80%;④三氧化二砷治疗1例难治复发进展期的MM患者在2个疗程后取得完全缓解。结论:①中年以上不明原因的骨痛、贫血、反复感染、白蛋白正常而白球蛋白比例降低、蛋白尿、血沉快等应考虑MM的可能,应进一步检查;②COP/CTHPOP化疗方案对初治MM有较好疗效;③三氧化二砷可用于治疗难治复发的MM患者。

Clincal Analysis on 19 Patients with Multiple Myeloma

Objective:To discuss the clinical features of multiple myleoma.Method:Retropective analysis was utilized.Result:Myloma might be with insidious onset and with wide range of symptoms and easily misdiagnosed.Most patients showed symptoms of bone pain and anemia.Monoclonal paraprotein on serum protein electrophoresis is mainly IgG and IgA paraprotein.Five patients with myeloma newly diagnosed was treated by COP/CTHPOP regimen,which effective rate was 80%.one relapsed and refractory patient with multiple myeloma achieved complete remission after two courses of treatment by Arsenic trioxide.Conclusion:Middle-age patients with the symptoms of bone pain,anemia,repeated infection,low rate of albumin and globulin but normal albumin,proteinuria,increased blood sedimentation,without any causes,should be with the possibility of myeloma and must be made the corresponding check.COP/CTHPOP regimen was also good for newly diagnosed patients with multiple myeloma.Arsenic trioxide can treat patients with relapsed or refractory multiple myeloma.
