腰椎后路融合手术术中自体血回输对患者的影响

目的探讨术中自体血回输对腰椎后路融合术患者术后血红蛋白变化以及住院时间的影响。方法将2002年1月-2005年12月122例行腰椎后路内固定并接受输血的患者分为实验组(66例)和对照组(56例),实验组采用术中自体血回收,对照组使用异体输血。比较2组病例的术中失血量、术后引流量、异体输血量、术后血红蛋白值和影响住院时间的因素,以及输血后并发症发生率。结果实验组患者平均输入异体血170ml,对照组患者平均输入异体血615ml,2组间有显著性差异(P<0.05);2组术后各时间点的血红蛋白值无显著性差异(P>0.05);2组患者住院时间无显著性差异(P>0.05),内固定节段数目与住院时间有显著性差异(P<0.05);实验组1例患者术后出现菌血症,对照组中4例出现输血过敏反应。结论采用自体血回输技术对患者术后血红蛋白变化及住院时间无明显影响,可以防止脊柱融合手术时因输各种异体血引发的并发症发生。

The effect of intraoperative drainage autologous blood transfusion on patients with posterior lumbar fusion surgery

Objective To investigate the effect of intraoperative drainage autologous blood transfusion on postoperative hemoglobin change and length of stay in posterior lumbar fusion surgery cases. Methods From January 2002 to December 2005, 122 patients who underwent the posterior lumbar internal fixation surgery and accepted blood transfusion were divided into the experiment group (66 cases) and the control group (56 cases). The former group received interoperation drainage autologous blood transfusion and the later group adopted homologous blood transfusion. The blood loss during operation, the amounts of drainage and homologous blood transfusion post operation, the hemoglobin values post operation, the influence factors of hospital stay and the incidence rate of complications of blood transfusion were compared between the two groups. Results The mean amount of variant blood in the experiment group was 170 ml that was lower than 615ml in the control group, showing a significant difference between the two groups (P<0.05). There was no difference between the two groups in hemoglobin value change of each time point and hospital stay (P>0.05). However, There was a significant difference between the amount of internal fixation spinal segments and the hospital stay (P<0.05). 1 case occurred bacteremia in the experiment group and 4 cases occurred blood transfusion anaphylaxis in the control group after the operation. Conclusions The autologous blood transfusion has no affection on the change of hemoglobin and hospital stay, which can avoid complications followed by the homologous blood transfusion in the spinal fusion surgery.
