中风病(脑梗死)急性期中医药疗法的运用及效益研究

目的观察中医药疗法与内科常规输液疗法治疗中风病(脑梗死)急性期的临床疗效及医疗费用。方法依据既定标准选择200例中风病(脑梗死)急性期患者,按就诊顺序随机分为观察组100例和对照组100例。观察组用中药汤剂辨证论治、针灸、中药专药3种中医特色疗法进行治疗;对照组给予内科静脉输液等常规方法治疗。2组均以2周为1个疗程。运用神经功能缺损评分(NIHSS)量表及中医症状评分量表对患者进行疗效评价,根据住院费用进行医疗成本评价。结果观察组总有效率93%,对照组86%;中医症状评分观察组总有效率94%,对照组85%。2组比较均有显著性差异(P均<0.01)。观察组治疗后基于NIHSS量表及中医症状评分量表的指标均较治疗前明显好转,且明显优于对照组。观察组治疗费用(1 844±284)元,对照组(4 025±375)元,2组比较有显著性差异(P<0.05)。结论中医药疗法治疗中风病(脑梗死)急性期疗效好,医疗费用低,患者依从性好,可为国家调整医保政策提供技术支持。

Study on the application of the therapy with traditional Chinese medicine for stork in acute stage and its medical cost

Objective It is to observe the clinical curative effect and medical cost of therapy of traditional Chinese medicine and department of internal medicine in conventional infusion therapy in the treatment of stroke(cerebral infarction) during acute phase.Methods According to established criteria 200 cases of stroke(cerebral infarction patients in acute stage) were chosen and randomly divided into the observation group(n=100) and control group(n=100).Observation group was treated with three characteristic of TCM therapy including Chinese medicine decoction based on syndrome differentiation,acupuncture,herbal medicine specifically drug for treatment;control group was given conventional methods such as medical intravenous infusion therapy.Two weeks was a course of treatment in both groups.NIHSS scale and TCM symptoms rating scale were used to evaluate the patients' curative effect,and medical cost evaluation was done according to the hospitalization expense.Results The total effective rate of NIHSS in the observation group was 93% in observation group and 86% in control group;the total effective rate of TCM symptom score in observation group was 94% and 85% in the control group,there was significant difference between the two groups(P<0.01).The indexes in the observation group after treatment based on the NIHSS scale and TCM symptoms rating scale were obviously better than that before treatment and in control group.The treatment costs of observation group was(1 844±284) yuan,and that of control group were(4 025±375) yuan,the difference between the two groups was significant(P<0.05).Conclusion Traditional Chinese medicine therapy for stroke(cerebral infarction) in the acute phase has good curative effect and low medical cost,with good patient medical compliance,this can provides technical support for national adjustment medicare policy.
