腹腔镜治疗盆腔脓肿32例临床分析

目的:探讨腹腔镜手术治疗盆腔脓肿的临床效果及可行性.方法:回顾分析32例因盆腔脓肿行腹腔镜手术患者的临床资料.手术范围根据病变范围、患者年龄、一般状况等全面考虑.输卵管积脓、卵巢积脓患者,以切除病灶为主;年龄较大、双侧附件受累或附件脓肿反复发作的患者,可行全子宫及双附件切除术.结果:术后3d患者体温均恢复正常,临床症状迅速消失.术后24 ～36 h恢复排气,3～5d复查白细胞恢复正常.无一例术后感染及复发,术后7d治愈出院.结论:腹腔镜手术更精确、彻底、微创,患者痛苦小,术后康复快,住院时间短,更利于保护生育功能,患者易接受,具有良好的临床效果及可行性,值得推广应用.

The clinical analysis of treatment of pelvic abscess with laparoscopy: with a report of 32 cases

Objective:To evaluate the clinical effect and feasibility of laparoscopy in treatment of pelvic abscess.Methods:The clinical data of 32 patients with pelvic abscess who underwent laparoscopy were analyzed retrospectively.The range of operation based on comprehensive consideration of lesion range,age and general condition.The patients with pyosalpinx and pyo-ovarium underwent focal excision,in addition,the elderly patients with bilateral accessory lesions or recurrent accessory abscess underwent hysterectomy and bilateral adnexectomy.Results:All the operations were performed successfully without severe complications.The temperature returned to normal on 3rd day after operation,the clinical symptoms rapidly disappeared,the exhaust recovered after 24-36 h,the white cells recovered to normal on 3rd-5th day,on 7th day after operation patients were cured.Conclusions:Laparoscopy for pelvic abscess is precise,radical,advantageous to the protection of reproductive function,acceptable to patients and mini-invasive,has advantages such as quick recovery and short hospital stay.It is worth popularizing with good clinical effect and feasibility.
