合并血小板减少的肺结核患者血小板氧化应激研究

摘    要:目的:研究合并血小板减少的肺结核患者血小板氧化应激情况。方法:随机选取初治肺结核患者40例(血小板减少20例(实验1组)、正常20例(实验2组))和体检健康者20例(对照组)。用流式细胞学检测血小板内活性氧簇(ROS)的表达。提取血小板蛋白质;用比色法检测血小板内丙二醛(MDA)、超氧化物歧化酶(SOD)和谷胱甘肽过氧化酶(GSH-PX)的浓度。结果:实验1组表达二氯荧光素(DCF)阳性ROS的血小板比例为(43.7±3.2)%明显高于实验2组((2.7+0.9)%)和对照组((2.1±0.7)%)(P〈0.01);实验2组表达DCF阳性ROS的血小板比例与对照组比无统计学意义。实验1组血小板内MDA浓度明显高于实验2组和对照组(P〈0.01),而SOD和GSH-PX浓度则显著低于实验2组和对照组(P〈0.01);实验2组血小板内MDA、SOD和GSH-PX浓度与对照组比无统计学意义。结论:血小板处于氧化应激和过氧化损伤状态,可能是肺结核者合并血小板减少的原因。

Abstract:Objective:To investigate the oxidative stress of platelets from patients with pulmonary tuberculosis accompanied by thrombocytopenia. Methods : Forty patients with primary pulmonary tuberculosis(20 cases with thrombocytopenia(experimental group 1)and 20 cases with normal platelet count(experimental group 2)and 20 healthy volunteers(control group)were enrolled. The reactive oxygen species(ROS)in platelets was detected by flow cytometry. The total protein in platelets was extracted, and the concentrations of malondialdehyde(MDA), superoxide dismutase(SOD)and glutathione peroxidase(GSH-PX)in platelets were determined by means of colorimetry. Results:The proportion of dichlorofluorescin(DCF)positive ROS platelets from patients in the experimental group 1(43.7?3.2)% was more than that of experimental group 2(2.7+0.9)% and control group(2.1?0.7)%(P0.01),there was no statistical difference of DCF positive ROS platelets proportion between experimental group 2 and control group. The content of MDA in platelets from patients in the experimental group 1((23.7?4.3) μmoL/mg pro)was more than that of experimental group 2 and control group(P0.01),the contents of SOD((104.0?8.6)μmoL/mg pro)and GSH-PX((1346.5?161.2)μmoL/mg pro)were less obviously than experimental group 2 and control group(P0.01),there was no statistical difference between experimental group 2 and control group in the contents of MDA,SOD and GSH-PX. Conclusion:Platelets from patients suffer from oxidative stress and peroxidative injury,which is probably the reason of thrombocytopenia in patients with pulmonary tuberculosis.
