血浆纤维蛋白原D-二聚体术后检测应用探讨

目的探讨术后长期卧床患者进行D-二聚体（D-D）和纤维蛋白原（Fg）检测的临床意义。方法分别用透射比浊法和酶联免疫吸附试验法（ELISA）测定术后需长期卧床患者的Fg和D-D，比较二者与健康对照组、一般组及术前后的差异。结果一般组和卧床组术后第2天和第5天与健康对照组及术前的D-D和Fg比较均升高，差异均有统计学意义（P〈o．05）。卧床组在术后5dD-D和Fg均较一般组高，而且与卧床组第2天相比检测水平也升高，差异具有统计学意义（P〈0．05）。结论术后卧床患者及时检测Fg、D-D能有效监测静脉血栓的形成，使临床医生能及时采取有效的预防措施，避免并发症的发生。

Applicable value of postoperative measurement of plasma D-dimer and fibrinogen

Objective To explore the clinical significance of postoperative measurement of plasma D-dimer and fibrinogen. Methods Turbidimetry was applied to measuring plasma fibrinogen level, and ELISA applied to measur- ing plasma level of D-dimer in postoperative long-term bed patients （bed patients group）, other postoperative patients （common group） and healthy subjects （healthy control group）. The differences in plasma levels of D-dimer and fibrinogen in bed patients group were compared with those in healthy control group and in common group. The differences were also compared in bed patients group and common group before and after surgery. Results The D-dimer and fibrinogen levels in common group and bed patients group were significantly higher on day 2 and day 5 after surgery than those before surgery as well as in healthy control group （P〈0.05）. The D-dimer and fibrinogen levels were significantly higher on day 5 in bed patients group than those on day 2 in the same group as well as on day 5 in common group （P〈0.05）. Conclusion. Timely mornitoring of D-dimer and fibrinogen in postoperative long-term bed patients can monitor the formation of venous thrombosis effectively, and contribute to enforcement of effective preventive measures so as to avoid the occurrence of complications timely.
