自身抗体阴性的原发性干燥综合征行唇腺活检的临床分析

摘    要:目的 探讨如何提高自身抗体(抗SSA/SSB抗体)阴性原发干燥综合征(pSS)患者唇腺活检诊断的刚性率.方法 回顾性分析82例抗SSA/SSB抗体阴性疑似pSS患者,行同位素检查腮腺受损情况和进行唇腺活检进行确诊情况.结果 82例患者唇腺活检病理报告阳性51例(62.2%),其中ChisholmⅢ级17例,Ⅳ级34例.51例唇腺活检阳性组中有44例(86.3%)腮腺同位素检查为中度以上受损;31例唇腺活检阴性组中有15例(48.4%)腮腺同位素检查为中度以上受损;2组之间差异有统计学意义(P<0.01).结论 腮腺同位素检查受损在中度以上者,唇腺活检对自身抗体阴性pSS诊断的阳性率较高.
Abstract:
Objective To improve the diagnosis postive rate of primary Sjogren's syndrome (pSS) by labial salivary gland(LSG) biopsy in patients with negative autoantibodies(SSA/SSB autoantibodies). Methods Eightytwo suspected pSS inpatients with both negative SSA/SSB autoantibodies were analyzed retrospectively. The decreased parotid gland function was tested and the LSG biopsy was performed to confirmed diagnosis. Results There were fifty-one patients(62.2% ) were LSG biopsy positive, including 17 Chisholm Ⅲ and 34 Chisholm Ⅳ in histopathologic grading. And 44(86% ) were found Parotid gland function moderate/severe damaged in the 51 LSG positive patients, moreover 15(48% ) patients were found moderate/severe damaged in 31 LSG negtive patients, A chi-square test was used between the two groups, the difference was significant ( P < 0.01 ), Conclusion LSG biopsy might be sensitive in suspected pSS patients with negtive antoantibodies when the parotid gland function is moderate/severe damaged.
