基质金属蛋白酶组织抑制剂1在直肠癌组织中表达的临床意义

目的 探讨直肠癌组织中基质金属蛋白酶组织抑制剂1(TIMP-1)表达的临床意义.方法 采用免疫组织化学法检测TIMP-1在40例直肠癌组织、30例正常直肠组织中的表达,并分析其与直肠癌临床病理特征及预后的关系.结果 TIMP-1在直肠癌组织中表达率(62.5%)明显高于正常直肠组织(36.7%);TIMP-1阳性表达率与Dukes分期、淋巴结转移及远处脏器转移呈负相关,与分化程度呈正相关;TIMP-1阳性表达直肠癌患者的5年生存率(82.6%)明显高于阴性患者(52.1%),差异均有统计学意义(P＜0.05).结论 TIMP-1在直肠癌发生发展中起重要作用,检测TIMP-1对直肠癌侵袭转移的预测、预后评估及指导合理的综合治疗等具有重要的临床意义.

Abstract：

Objective To investigate the clinical significance and prognosis about expression of tissue inhibitor of metalloproteinase-1 (TIMP-1) in rectal cancer. Methods The expression of TIMP-1 in 40 samples of rectal cancers and 30 normal rectal mucosa was examined using immunohistochemistry. The relationship of their expression with some clinicopathological characteristics and prognosis was analyzed. Results The positive expression rate of TIMP-1 (62.5%) was higher in rectal cancer than normal rectal mucosa. Increased TIMP-1 expression was significantly correlated with tumor differentiation, Dukes stage and lymph nodes and distant metastasis. Compared with the cases of negative TIMP-1 expression, the patients with positive TIMP-1 expression had a significantly higher 5-year survival rate(P＜0.05). Conclusions TIMP-1 may play a key role in rectal carcinogenesis, tumor invasion and metastasis. TIMP-1 is a allergy indicator for diagnosis of rectal cancer with high specificity. The expression of TIMP-1 is directly correlated with the invasion and metastasis of rectal cancer and prognosis.
