摘    要:目的 探讨去骨瓣减压术治疗大面积脑梗死的有效性.方法 回顾性分析我院2006年1月至2009年6月58例因病情进行性加重而早期行去骨瓣减压术的急性大面积脑梗死患者临床资料.结果 58例中52例存活,6例死亡.52例存活者中38例随访6个月,智能均接近正常.术后7 d格拉斯哥昏迷量表评分(11.2±4.4)分,6个月日常生活能力评分(81.2±11.3)分.结论 去骨瓣减压术不仅可作为一种挽救患者生命的手术,而且多数存活病例恢复较好.合理选择手术适应证、及时把握手术时机以及充分手术减压是影响预后的重要因素.
Abstract:
Objective To investigate the validity of decompressive hemicraniectomy in massive cerebral infarctionl. Methods Totally 58 patients received early decompressive craniotomy because of deterioration after infarction. Results Fifty-eight patients had transtentorinl hernia due to massive cerebral infarction, among them 52 survived and 6 died. 52 survivals were followed up for 6 months and they had nearly normal intelligence and 38 of them lived independently. Conclusion Decompressive craniectomy can be a life-saving intervention to some of large cerebral infarction and functional recovery is probably associated with operation timing and extensive craniectomy.
