Gd—DTPA对鼻咽癌诊断的价值

目的：分析Ｇｄ－ＤＴＰＡ对鼻咽癌诊断的价值。材料与方法：４０例鼻咽癌患者行Ｇｄ－ＤＴＰＡ增强前后ＭＲＩ检查，其中放疗前１８例，放疗后局部复发（ＲＮＰＣ）２２例；对照组包括放疗后纤维化（ＲＦ）６例和炎性病灶２例。结果：Ｇｄ－ＤＴＰＡ增强ＭＲ检查能清晰显示鼻咽癌的侵犯范围和边界；ＲＮＰＣ与ＲＦ病灶增强后信号强度有显著性差异（Ｐ〈０．０１）；病灶无强化，可除外ＲＮＰＣ，有强化则不一定是ＲＮＰＣ，炎性病灶

Evaluation of Gd DTPA in the Diagnosis of Nasopharyngeal Carcinoma

Objective: The value of Gd DTPA in the diagnosis of nasopharyngeal carcinoma (NPC) was analysed.Materials and Methods:40 patients with NPC were studied with MRI before and after IV injection of Gd DTPA, 18 patients had untreated NPC, and 22 tumors had recurrence (RNPC). The control group was consisted of 6 patients with radiation fibrosis (RF) and 2 patients with inflammatory change.Results:Contrast enhanced MR enabled better identification of tumor extension. The difference of enhancement between RNPC and RF was more significant (P< 0.01 ). The mass lesions without enhancement couldn't be RNPC, but the enhancement of tumor is not specific and may be seen in inflammation.Conclusion: Gd DTPA enhanced MRI is recommended for tumors which are small or difficult to define the margin on plain MRI, and is very useful in the differential diagnosis of RNPC.
