| 摘 要： | 目的 探讨定量胰岛素敏感检测指数(QUICKI)对判断脑卒中患者预后的价值.方法 对67例脑卒中(伴糖尿病38例)患者的预后(发病1个月死亡或生存)、血糖及胰岛素水平进行观察,计算QUIC-KI;分析其血糖、胰岛素水平及QUICKI与预后的关系.结果 本组患者中死亡37例,存活30例.死亡患者的血糖[(11.64±4.14)mmol/L]、胰岛素水平[(41.21 ±23.58)mU/L]明显高于存活患者[(8.41±2.66)mmol/L、(25.90±24.00)mU/L],而QUICKI(0.115 ±0.011)显著低于存活者(0.129±0.015)(P＜0.05～0.01);伴有糖尿病的脑卒中患者中,死亡患者与存活患者的血糖、胰岛素水平差异无统计学意义;伴糖尿病的脑卒中患者中,QUICKI＜0.1181者死亡危险性明显高于QUICKI≥0.1181者(OR=5.87,95%C/:1.50～22.93).结论 QUICKI值可作为脑卒中患者,尤其是伴糖尿病的脑卒中患者判断预后的指标. |
