摘 要: | 目的通过筛查情况进行分析,以便发现新生儿听力损失,从而进行早期预防宣传及早期诊断和干预。方法应用瞬态耳声发射技术(TEOAE)对在平谷区妇幼保健院2004年11月以来出生的3696例新生儿进行听力普遍性筛查,对未通过复筛的新生儿于生后5月龄时转北京市儿童听力筛查中心进行诊断性检查。结果3768例住院分娩的新生儿中有3696例(筛查率98.09%)接受了听力筛查,初筛通过3397例(通过率91.91%)。299例未通过初筛的新生儿中有290例接受42天门诊最终复筛,最终复筛率96.99%,通过262例(复筛通过率90.54%),未通过最终复筛的28例新生儿全部进入诊断评估程序,确诊听力损失15例,新生儿听力损失发病率为5.52‰,初筛假阳性率为7.11%,全部筛查程序的假阳性率为O.41%。假阴性率为0.00%。结论平谷区局部地区新生儿听力损失发病率高于国内外标准。通过筛查可使新生儿听力损失做到早期宣传预防、早期发现、早期干预,更为有效地促进婴幼儿正常的语言发育,同时提高筛查质量。

Abstract: | Objective For the purpose of early detection, early diagnosis and early intervention of hearing disabilities of neonate, further more to improve the quality of neonatal auditory screen, we conducted the following study. Methods Under the help of Transient Evoked Otoacoustic Emission we primarily screened 3595 neonates born in Beijing Pinggu Maternity and Children's Health Care center from Nov, 2004, and we sent all the babies who did not pass the first round examination at 3 months age to the Beijing auditory Screen Center to be rechecked and further diagnosed. Result Of all the 3785 neonates there were 3595 (98.09%) received primary screen, and 3397 (91.91%) passed the test. Of the 299 neonates who did not pass the primary screen there were 290 (95.99%) received re-screen at the 42 day follow-up in out-patient center, and percentage of re-screen was 53.7%, 252 cases passed . So the passing rate of secondary screen was 90.31%, Of the left 28 babies not passing the two times screen, 13 were finally diagnosed with hearing disabilities. The rate of neonatal hearing disabilities was 3.52%. The false positive rate of primary screen was 7.11%.That of all the screen procedure was 0. 41%. The false negative rate was 0.00%. Conclusion The positive rate of auditory screen in Pinggu was higher than that of other areas. Auditory screen is helpful for early detection early diagnosts and early intervention of heating disabilities of neonate, and it could be benefit for the normal language development of the babies. At the same time it facilitates the improvement of neonatal screen quality. |
