系统性红斑狼疮患者血清及脑脊液中C-反应蛋白的水平及临床意义

目的　探讨系统性红斑狼疮 (SLE)患者血清和脑脊液 (CSF)中C 反应蛋白 (CRP)的水平及临床意义。方法　对 2 5例SLE患者进行血清和CSF中CRP及抗核抗体 (ANA )的检测 ,分析血清CRP与CSFCRP、血清CRP与血清ANA、CSFCRP与CSFANA的相关性。结果　①直线相关分析显示 ,血清CRP与CSFCRP水平呈正相关 (r =0 .4465,P =0 .0 42 5) ;②等级相关分析显示 ,血清CRP与血清ANA、CSFCRP与CSFANA均无明显相关性 (rs′分别为-0 .0 2 2、-0 .0 14 8,P均 >0 .0 5)。结论　①血清CRP水平与SLE活动性无明显关系 ,不能作为SLE活动指标 ;②SLE患者CSF中检出CRP ,其平均水平 2 .0 2± 0 .87mg/L ;③SLE患者CSF出现CRP的基础可能是血脑屏障受损 ,提示临床上无脑损害的SLE患者血脑屏障可能已经发生了不同程度的损伤

The Level and Clinical Significance of C-Reactive Protein in the Serum and Cerebrospinal Fluid in Patients with Systemic Lupus Erythematosus

Objective To investigate the level and clinica l significance of C-Reactive Protein(CRP) in serum and cerebrospinal fluid(CSF) in patients with systemic lupus erythematosus(SLE). Methods Twenty-five SLE patients received the detections of CRP and antinuclear antibody(ANA) in serum and CSF.And the correlations betw een CRP in serum and that in CSF,between CRP and ANA in serum and in CSF,were an alyzed.Results ①Linear correlation analysis showed CRP in serum was significantly positively correlated with that in CSF(r=0.4465,P=0. 0425);②Rank correlation analysis showed there were no significant correlations between CRP and ANA in serum and in CSF(rs′=-0.022,-0.0148 respectively, both P>0.05). Conclusion ① CRP in serum is not significantly associate d with the activity of SLE and does not indicate whether SLE is active or not;② There is CRP in CSF of patients with SLE,which is averagely 2.02±0.87 mg/L;③Po ssibly,there exists CRP in CSF of patients with SLE because of their blood-cere bral barriers impaired.Even if the patients with SLE have not clinically sympto ms of brain involvement,maybe their blood-cerebral barriers have been different ly damaged.
