摘    要:目的:分析肿瘤患者血脂水平的影响因素以及血脂与相位角(PA)、体脂百分比(PBF)的相关性,为肿瘤患者营养管理和研究方向提供参考。方法:收集2020年12月至2022年04月我院收治的159例肿瘤患者一般临床资料、患者主观整体营养评估量表(PG-SGA)评分、用药史、既往病史、个人史、入院后血脂、人体成分分析结果。通过比较不同临床特征患者血脂水平和PBF的差异,血脂、PG-SGA评分与PA、PBF之间的相关性,分析肿瘤患者血脂与PA、PBF之间的关联和影响。结果:不同瘤种之间甘油三酯(TG)和PBF存在差异(P<0.05);女性、低龄和行为状况(PS)评分低的患者,总胆固醇(TC)、TG要高于男性、高龄和PS评分高的患者(P<0.05),吸烟患者有更低TG和载脂蛋白A(Apo-A)(P<0.05),饮酒患者高密度脂蛋白(HDL)更低(P<0.05)。中度营养不良患者TC和低密度脂蛋白(LDL)水平最低(P<0.05),TG随着营养状态恶化单调下降。将抗肿瘤治疗措施按血脂异常的风险进行分组比较,各风险组未见血脂水平存在差异。PA与PG-SGA呈显著负相关(r=-0.425,P<0.001),与TC、TG、LDL呈正相关(P<0.05)。PBF与TC、TG、LDL呈正相关(P<0.001)。控制性别后,偏相关检验PBF与TC、TG、LDL仍呈正相关(P<0.05)。结论:肿瘤患者血脂受多种因素影响,TC、TG、LDL与PA、PBF正相关,肿瘤患者应避免血脂过低。脂代谢调节治疗和抗肿瘤药物对于血脂异常的影响,有待进一步研究。

Abstract:Objective:To investigate the influencing factors of blood lipids and the correlation of blood lipids with phase angle (PA) and percent body fat (PBF) in tumor patients,and to provide reference for nutritional management and research direction of tumor patients.Methods:From December 2020 to April 2022,totally 159 tumor patients admitted to our hospital were selected.Clinical data,patient generated subjective global assessment (PG-SGA) score,medication history,past medical history,personal history,results of blood lipid examination and body composition analysis were collected.The differences of blood lipids and PBF in patients with different clinical features were compared and the correlation of blood lipid level and PG-SGA with PA as well as PBF were analyzed.Results:Triglyceride (TG) and PBF of different tumor patients were significantly different (P<0.05).The total cholesterol (TC) and TG of female,young and low performance status (PS) score patients were higher than those of male,old and high PS score patients (P<0.05).Compared with non-smoking patients,smokers had lower TG and apolipoprotein A (Apo-A) (P<0.05).What's more,patients with long-term alcohol consumption had lower high density lipoprotein (HDL) than non-drinking patients (P<0.05).The levels of TC and low density lipoprotein (LDL) in patients with moderate malnutrition were the lowest (P<0.05).TG decreased monotonously with the deterioration of nutritional status.There was no difference in blood lipid levels among different risk groups after grouping antitumor treatment measures according to the risk of dyslipidemia.PA was negatively correlated with PG-SGA (r=-0.425,P<0.001),whereas it was positively correlated with TC,TG and LDL (P<0.05).PBF was positively correlated with TC,TG and LDL (P<0.001).Notably,after excluding the influence of gender,the results of partial correlation analysis demonstrated that PBF was still positively correlated with TC,TG and LDL (P<0.05).Conclusion:The blood lipid of tumor patients is affected by many factors.TC,TG and LDL are positively correlated with PA and PBF in tumor patients.Tumor patients should prevent hypolipidemia.The effects of lipid metabolism regulation therapy and antitumor drugs on dyslipidemia should be further studied.
