膀胱子宫内膜异位症诊疗分析

目的 分析并总结膀胱子宫内膜异位症的诊断和治疗方法.方法 回顾性分析2003-2013年我院收治的14例膀胱子宫内膜异位症患者的临床资料及诊疗方法.结果 14例患者平均年龄（40±5.5）岁,主要临床表现为周期性下腹痛,尿频、尿急、尿痛及周期性肉眼血尿;13例行腹腔镜下膀胱部分切除术,术后均辅以促性腺激素释放激素激动剂药物治疗,全部患者症状得以缓解,随访至今未见复发.结论 膀胱子宫内膜异位症根据典型临床表现,超声、核磁等影像学检查及膀胱镜检查可做出初步诊断;腹腔镜下膀胱部分切除术联合促性腺激素释放激素激动剂药物治疗是膀胱子宫内膜异位症的有效、合理的治疗方案.

Diagnosis and treatment of bladder endometriosis

Objective To analyze the diagnosis and treatment of bladder endometriosis.Methods Clinical data about 14 bladder endometriosis patients admitted to our hospital from 2003 to 2013 were retrospectively analyzed.Results The average age of the 14 patients was 40 ± 5.5 years.Their main clinical manifestations were periodical hypogastralgia,urgency and frequency of micturition,urodynia,and periodical gross hematuria.The symptoms were alleviated in 13 patients after laparoscopic partial bladder resection and GnRH-a therapy with no recurrence during the follow-up.Conclusion Diagnosis of bladder endometriosis can be established according to its typical clinical manifestations,ultrasonography,pelvic MRI and cystoscopy findings.Laparoscopy partial cystectomy in combination with GnRH-a therapy is an effective and rational treatment modality for bladder endometriosis.
