彩色多谱勒能量图在断指再植中的应用价值

摘    要:
目的探讨彩色多谱勒能量图在断指再植微血管检查中的价值。方法采用ASU-3000型彩色超声诊断仪,探头5~8mHz,对26条不同名称再植手指微血管吻合端进行彩色多谱勒及能量图检查。观察其周围血流信号数量及血流动力学改变。结果1条右环指桡侧暴露不充分,未检测到其血流频谱,其余25条再植手指血管吻合端彩色多谱勒血流信号(CDFI)检出率为72%,彩色多谱勒能量图血流信号(CDEI)检出率为100%,断指再植血管早期成活诊断符合率100%,断指再植术后远期功能恢复预测敏感性为72%,CDFI、CDEI间的检出率、符合率、敏感性比较均无显著性差异(P>0.05)。结论彩色多谱勒能量图准确提示临床断指再植血管早期成活情况,其检出率、敏感性均超过彩色多谱勒血流图,是检测微血管血流的首选,尤其对指导断指再植临床治疗及术后远期功能恢复程度预测具有重要意义。

Abstract:
Objective To evaluate the color Doppler energy imaging(CDEI)in the assessment of the blood supply in replantation of severed finger.Methods The anastomosis sites of 26 different blood vessels of the replanted finger were examined on a color Doppler AUS-3000 color ultrasonator,with a 5-8 mHz probe.The intensity of blood stream and hemodynamics were studied.Results A lateral blood vessel of the ring finger could not be detected.The positive rate of the rest 25 blood vessels was 72%on color Doppler flow signal(CDFI),and 100%on CDEI,severed finger replantation blood vessel is it diagnose 100%coincidence rate to survive in early days,severed finger plant long-term function is it predict sensitiveness 72 %,CDFI,examining personning who happen among the CDEI,coincidence rate,sensitiveness have a significance difference to resume after the skill again(P > 0.05).Conclusion CDEI cable to exaxtable register for clinical severed finger plant blood vessel survive the situation in early days,its examining appearing rate,sensitiveness more than CDFI,measure first-selection using blood capillary of blood flow,especially to instruct severed finger plant clinical treatment and long-term function resume degree predict significant after the skill again.
