| 摘 要： | 目的:探讨高位肠瘘病人肠液全封闭回输结合EN,对肠瘘治疗的影响. 方法:收集自2004年10月至2006年12月期间收治的高位肠瘘病人21例,所选病人均符合以下条件:瘘口均在空肠近端50 cm内;均为外伤或手术后10～15 d后,每天漏出的肠液>1 000 mL;采取手术或气囊堵塞导管的方法,建立瘘口远、近端肠液回输引出和输入必备的管道;检测肠液回输前后血清蛋白质水平,对比肠液回输前后糖、脂肪和氮的吸收系数. 结果:肠液回输后,病人的血清蛋白质水平明显增加(P<0.01或P<0.05),糖、脂肪和氮的吸收系数明显上升. 结论:高位肠瘘病人在肠液全封闭条件下回输后,EN物质中,糖、脂肪和氮的吸收系数明显上升,血清蛋白质明显升高.避免漏出的肠液回收后再处理,减少了污染的机会.
