| 摘 要： | 目的：探讨上尿路移行细胞癌术后再发膀胱癌的特点。方法：对98例上尿路移行细胞癌术中后再发膀胱癌24例进行临床回顾性分析。结果：上尿路移行细胞癌行肾输尿管膀胱袖状切除术后膀胱癌再发率低于肾输尿管切除者，两组比较差异有极显著性意义（P＜0．01）；上尿路移行细胞癌合并膀胱癌、肿瘤恶性程度高及输尿管下段癌其术后膀胱癌的再发率高，再发时间多在术后3年以内。结论：肾输尿管膀胱袖状切除术是上尿路移行细胞癌最有效的治疗方法，术后定期复查膀胱镜和膀胱抗癌药灌注很重要。

| Abstract: | Objective To study the recurrent bladder carcinoma after resection of upper urinary tract transitional cell carcinoma.Method 24 cases of recurrent bladder carcinoma out of 98 cases of postoperative upper urinary tract carcinoma were analyzed.Results The postoperative recurreut rate of bladder carcinoma with total nephroureterectomy combined with excision of bladder cuff was lower than that of nephroureterectomy( P <0.01);In the cases of upper urinary tract carcinoma associated with bladder carcinoma,higher grade malignancy and lower ureter carcinoma the recurrence rate was higher and usually occurred within 3 years.Conclusions The most effective treatment is total nephroureterectomy combined with excision of bladder cuff, periodic cystoscopy and vesical instillation of anticancer drug is necessary. |
