老年心肌梗死的特点及临床分析

目的探讨老年心肌梗死的临床特点,以指导医生的诊断与治疗。方法回顾性分析2010年1月～2011年12月收治的35例老年心肌梗死患者的临床资料。结果 35例患者的首发症状以胸痛最为常见。梗死部位为前侧壁者7例,前间壁者7例,广泛前壁者10例,下壁者9例,心内膜下者2例。并发心力衰竭15例,脑血管意外9例,心律失常20例。经治疗,好转29例,好转率为82.86%;死亡6例,病死率为17.14%。结论老年急性心肌梗死发病症状不典型,易误、漏诊,临床医生对高危及疑似患者均应保持警惕,以提高诊断率,做到早诊断早治疗,改善患者预后。

Characteristics and clinical analysis of myocardial infarction in the elderly

Objective To probe the clinical characteristics of myocardial infarction in the elderly for guiding diagnosis and treatment.Methods The dates of 35 patients with myocardial infarction in the elderly that collected from Jan 2010 to Dec 2011 were analyzed retrospectively.Results In 35 patients,chest pains was the most common first symptom.Anterolateral infarction 7 cases,anteriorseptal infarction 7 cases,extensive anterior infarction 10 cases,inferior wall infarction 9 cases,subendocardial infarction 2 cases.Complicated with cardiac failure 15 cases,cerebrovascular accident 9 cases,arrhythmia 20 cases.After treatment,improvement 29 cases(82.86%),death 6 cases(17.14%).Conclusion The symptoms of acute myocardial infarction in the elderly are atypical.It cause erroneous diagnosis and misdiagnosis easily.So clinical doctors should pay attention to high risk or suspected patients for improving the correctness of diagnosis.
