扁桃体切除术和腺样体刮除术治疗儿童阻塞性睡眠呼吸暂停综合征疗效观察

目的:观察扁桃体切除术和腺样体刮除术治疗儿童慢性阻塞性呼吸暂停综合征(OSAS)的疗效。方法:选择符合OSAS诊断标准的43例儿童作为治疗组,进行扁桃体切除术和(或)腺样体刮除术治疗;同时选择44例正常儿童作为正常对照组。观察治疗组儿童手术前后多导睡眠图(PSG)的主要指标,测定治疗组儿童手术前后血浆中生长激素(GH)的含量及正常对照组儿童血浆中GH的含量。结果:与手术前比较,治疗组儿童PSG的各项指标中血氧饱和度上升(P<0.05),其他指标下降(P<0.01)。治疗组儿童手术前血浆生长激素含量低于正常对照组,两组比较有显著差异(P<0.05);术后3个月生长激素含量升高,与对照组比较无明显差异。结论:扁桃体切除术和腺样体刮除术是治疗儿童OSAS的有效方法。

Effect of Tonsillectomy and Adenoidectomy in Children with Obstructive Sleep Apnea Syndrome

Objective:To observe the effect of tonsillectomy and adenoidectomy on obstructive sleep apnea syndrome(OSAS) of children. Methods:43 children with OSAS were selected as treatment group treated with tonsillectomy and adenoidectomy,and 47 normal children served as normal control group. PSG indexes,growth hormone (GH) in plasma of treatment group were observed before and after surgery and GH in plasma of normal control group were observed. Results:Compared with PSG indexes of preoperative children,postoperative oxygen saturation of treatment group was increased (P<0.05),the other indexes were reduced (P<0.01). Preoperative GH level in plasma of treatment group was lower than that of normal control group and there is significant difference between two groups. GH level in plasma of treatment group was increased remarkablely 3 months after surgery,and there was no significant difference between normal control group and treatment group. Conclusion:Tonsillectomy and adenoidectomy are the effective therapy in children with OSAS.
