踝关节骨折合并下胫腓分离内固定治疗分析

目的探讨踝关节骨折合并有下胫腓分离的治疗,对于下胫腓分离应如何处理。方法收治65例踝关节骨折合并有下胫腓分离的患者,随机分为治疗组31例,和观察组34例。治疗组同时固定踝关节和下胫腓联合,观察组仅固定踝关节。结果随访12～56个月,平均34个月。按Kofed评分法评定:治疗组优10例,良13例,可4例,差1例;观察组优12例,良9例,可7例,差2例。结论根据骨折的类型和软组织条件,踝关节骨折合并有下胫腓分离时只要内外踝获得坚强固定,下胫腓可不需要再固定。

Ankle Fracture Merge the Shank Phil Separation in Fixed Treatment Analysis

Objective Aim to explore ankle fracture combined with inferior tibiofibular joint Separation treatment, syndesmosis separation and how we should deal with. Methods Admission of 65 cases of ankle joint fracture combined with inferior tibiofibular separated patients, were randomly assigned to treatment of 31 cases, and Observer Group of 34 cases.Treatment and fixation and syndesmosis in ankle joint, observer group only fixed ankle. Results Follow-up 12-56 months, an average of 34 months.By Kofed score rating： The treatment group was excellent in 10 cases, good in 13 cases, 4 cases, and poor in 1 cases; the observation group was excellent in 12 cases, good in 9 cases, in 7 cases, and poor in 2 cases. Conclusion Based on type of fractures and soft tissue conditions, ankle fracture combined with inferior tibiofibular joint separation whenever the internal or external malleolus has strong fixed, syndesmosis don＇t need to be fixed.
