早期联合应用血必净注射液和机械通气治疗ALI/ARDS疗效观察

目的:评价早期联合应用血必净注射液和机械通气治疗急性肺损伤(ALI)/急性呼吸窘迫综合征(ARDS)患者的疗效。方法:将40例ALI/ARDS患者按随机原则分为对照组与治疗组,每组20例。对照组给予机械通气、抗生素、对症及营养支持治疗;治疗组在对照组治疗基础上加用血必净注射液50mL静脉滴注,12h1次,连用7d。比较2组患者治疗前和治疗后5d的血白细胞计数(WBC)、C-反应蛋白(CRP)、气道压力、动脉血气以及成功撤机率、重症加强治疗病房(ICU)病死率。结果:治疗前2组患者的气道压力、动脉血气、WBC、CRP比较差异均无统计学意义(P均>0.05);治疗后5d治疗组患者的气道压力、动脉血气、CRP改善程度均显著优于对照组(P均<0.05),撤机成功率显著高于对照组(80%比50%,P<0.05),ICU病死率显著低于对照组(20%比50%,P<0.05)。结论:早期联合应用血必净注射液和机械通气有利于ALI/ARDS患者撤机拔管,降低机械通气患者的ICU病死率。

Efficacy Observation of Xuebijing Injection Combined with Mechanical Ventilation in the Treatment of ALI/ARDS

OBJECTIVE:To evaluate clinical efficacy of Xuebijing injection combined with mechanical ventilation(MV)in the Treatment of ALI/ARDS. METHODS:Forty patients with ALI/ARDS were randomly divided into control group and treatment group(n=20). Both groups were given MV,antibiotics,symptomatic and nutrition support treatment,and treatment group were additionally treated with 50 mL Xuebijing injection every 12 hours for 7 days. The differences between two groups were compared in white blood cells(WBC),Creactive protein(CRP),airway pressure,arterial blood gas,ventilator weaning and mortality in intensive care unit(ICU). RESULTS:There was no statistical significance in difference between two groups before treatment in airway pressure,arterial blood gas,WBC,CRP(P>0.05). Five days after treatment,the improvement of airway pressure,arterial blood gas,CRP,ventilator weaning rate(80% vs. 50%)in treatment group were superior to control groups(P<0.05);the mor- tality in ICU of treatment group was obviously lower than that of control group(20% vs. 50%,P<0.05). CONCLUSION:Xuebijing injection combined with mechanical ventilation can contribute to ventilator weaning and lower the mortality of patients treated with MV in ICU.
