子宫肌瘤子宫全切术后卵巢内分泌功能的变化与研究

目的 探讨子宫肌瘤子宫全切术后卵巢内分泌功能的变化情况.方法 选取2008年4月～2011年3月入住某院妇科并接受治疗的子宫肌瘤患者共66例,选取在其他科室就诊的无内分泌系统疾病,月经、子宫卵巢均正常,年龄结构相近的妇女20例为对照组.观察组所有患者均行常规B超、X光片以及探测宫腔检查,当确诊为子宫肌瘤,即行子宫全切术进行治疗.观察组在术前月经d5及术后1、3、7月,对照组在月经后d5进行FSH(促卵泡激素)、LH(黄体生成素)、PRL(催乳素)、E2(雌激素睾酮)等几种激素的免疫测定,同时对观察组术前及术后1、3、7月的卵巢衰退症状进行统计分析.结果 术前FSH(促卵泡激素)、LH(黄体生成素)、PRL(催乳素)3种激素水平与对照组相比不存在显著性差异,术后,各类激素水平均出现显著性升高,在术后7个月时测量值最高,E2的水平在术前术后均处于较高水平.观察组患者术后出现盗汗,脱发,阴道干涩等症状的几率显著增加,从术前的39.4％增加到术后7个月的69.7％.结论 行子宫切除术的患者在术后都不同程度地出现了卵巢衰退迹象,且FSH(促卵泡激素)、LH(黄体生成素)、PRL(催乳素)3种激素水平与术后时间基本呈现正相关关系.

Change and studies of ovarian function after uterine fibroids hysterectomy

OBJECTIVE To investigate the changes of ovarian function after total hysterectomy due to uterine fibroids.METHODS From April 2008 to March 2011,a total of 66 cases treated for uterine fibroids in our hospital were selected,20 cases without endocrine system diseases in the other sections,whose menstruation,ovary and uterus were normal,age structure was similar in the control group.All patients in the observation group underwent conventional B-ultrasound,X-rays,and the detection of uterine examination,the cases of ultimate diagnosed uterine fibroid were conducted on hysterectomy for treatment.In observation group,the preoperative 5th day and postoperative 1 st,3rd and 7th months in the observation group,the fifth day after menstruation in the control group,FSH(follicle-stimulating hormone),LH(luteinizing hormone)PRL(PRL),E2(several hormones estrogen testosterone)and other immunoassays were detected,and symptoms of ovarian recession before treatment and postoperative 1 st,3rd and 7th months were observed in the observation group.RESULTS Compared to the control group,FSH(follicle-stimulating hormone),LH(luteinizing hormone),PRL(prolactin)and three kinds of hormones did not exist significant differences.After operation,various hormone were significantly increased,which were the highest in postoperative 7th month,E2 level before and after surgery was at a high level.The patients had a greater probability of night sweat,alopecia,vaginal dryness and other symptoms from preoperative 39.4% to postoperative 69.7%.CONCLUSION Patients after hysterectomy surgery have the varying degrees of ovarian signs of recession.Three types of hormone levels of FSH(follicle-stimulating hormone),LH(luteinizing hormone),PRL(prolactin)are basically positively correlated with the postoperative time.
