慢性乙型肝炎患者肝功能状况与胃黏膜炎症的相性研究

目的探讨慢性乙型肝炎患者的肝功能改变与胃黏膜炎症程度的相关性。方法选择某院自2009年1月～2010年12月收治的初次发病慢性乙型肝炎患者35例,行胃镜检查并取胃黏膜做病理检查,与其肝功能、HBV-DNA定量、凝血酶原时间(PT)进行相关性分析。结果慢性乙型肝炎患者的胃黏膜炎症主要发生在胃窦部,病理显示以轻度及中度慢性炎症为主;血清ALT、AST、HBV-DNA在不同程度的3组胃炎患者中差异无统计学意义(P﹥0.05);而ALB、PT 3组间比较差异有统计学意义(P﹤0.05),PT延长与慢性乙肝患者胃黏膜炎症程度相关。结论慢性乙型肝炎患者胃窦炎发病率较高,胃黏膜的炎症程度与肝功能损伤程度相关,PT可以作为胃黏膜炎症程度的预测指标,指导临床治疗。

Study on the correlation of liver function with chronic hepatitis B and gastric mucosal inflammation

OBJECTIVE To investigate the correlation of liver function with chronic hepatitis B and gastric mucosal inflammation.METHODS 35 patients with chronic hepatitis in our hospital from January 2009 to December 2010 were chosen.We performed endoscopy and gastric mucosal pathological examinations,liver function,HBV-DNA quantification,prothrombin time(PT)for patients.RESULTS The gastric mucosal lesions increased with the degree of liver function,and pathology showed mild and moderate chronic inflammation;There was significant difference in the ALB and PT in the liver function of the three groups(P﹤0.05).PT was the extension of gastric mucosal lesions in patients with chronic hepatitis B predictors.CONCLUSION The incidence of gastric inflammation for chronic hepatitis B patients was higher.The level of extent of gastric mucosal inflammation and liver lesions is closely related.Prothrombin time may be used as a predictor for the degree of gastric mucosal inflammation.
