血尿酸和脂蛋白-α水平与冠脉病变程度的关系

摘    要:目的探讨血尿酸(SUA)和脂蛋白-aLp(a)]水平与冠状动脉病变严重程度的相关性。方法 224例患者根据选择性冠状动脉造影(CAG)结果分为冠心病组121例,正常对照组103例,再根据Gensini评分系统将冠心病组分为2组,其中Gensini积分﹤30分66例为冠心病A组,Gensini积分≥30分55例为冠心病B组;再根据冠状动脉病变支数分为单支病变组46例,双支病变组46例,多支病变组29例。分别测定其SUA和Lp(a)水平,并分析二者与冠心病及冠脉病变严重程度的相关性。结果冠心病组SUA和Lp(a)水平明显高于正常对照组,冠心病B组SUA和Lp(a)的水平明显高于冠心病A组,双支病变组和多支病变组之间比较冠脉病变支数越多,SUA和Lp(a)水平就越高,单支病变组与对照组比较SUA和Lp(a)的水平差异无统计学意义(P﹥0.05)。SUA与Gensini积分相关系数为0.451,Lp(a)与Gensini积分相关系数为0.413。结论冠心病组SUA和Lp(a)水平高于正常对照组且冠心病患者SUA和Lp(a)水平的高低与冠脉病变严重程度呈明显正相关。

Abstract:OBJECTIVE To investigate the relationship between serum uric acid (SUA) , lipoprotion-a (LP-a) and the severity of coronary artery. METHODS According to coronary angiography, two hundred twenty-four patients were divided into two groups: the coronary heart disease (CHD) group (n=121) and non-CHD group (control group) (n=103). The CHD group were further divided into two subgroups according to accumulated points of Gensini scoring system: A group (n=66) of the Gensini score ﹤30 and B group (n=55) of the Gensini score ≥30. The CHD group were also further divided into three groups according to the number of vessel disease: one vessel disease group (n=46); two vessels disease group (n=46) and three vessels disease group (n=29). The level of SUA and LP-a were measured, and to analyze the relation between SUA and LP-a and the severity of coronary artery. RESULTS The levels of SUA and LP-a in CHD group were significantly higher than those of control group. The levels of SUA and LP-a in CHD B group were significantly higher than those of control group. The higher levels of SUA and LP-a had more the number of vessel disease (P﹤0.05). There was no significant difference in the SUA and LP-a between group A and group B. The coefficient correlation was 0.451 between SUA and the severity of coronary artery, and the coefficient correlation is 0.413 between LP-a and the severity of coronary artery. CONCLUSION The levels of SUA and LP-a in CHD patients were obviously positive correlation to the severity of coronary artery.
