血清胱抑素 C 及脂联素水平变化与糖尿病肾病的相关性研究

目的探讨检测血清胱抑素 C(Cys C)及脂联素(APN)水平在2型糖尿病肾病(DN )发生、发展机制中的作用及临床意义.方法选择2型糖尿病(T2DM )患者105例,根据尿微量清蛋白排泄率(UAER)分为3组:正常蛋白尿组35例,微量蛋白尿组35例,大量蛋白尿组35例;另选35例健康体检者为健康对照组.观察所有研究对象的尿微量清蛋白(mAlb)、Cys C 、APN 、空腹血糖(Fib)、糖化血红蛋白(HbAlc)、尿素氮(BUN )和三酰甘油(TG).结果3组患者血清 Cys C 水平分别为(1.72±0.57)、(2.49±0.86)、(3.97±1.20)mg/L ,明显高于健康对照组的(0.91±0.25)mg/L ,差异均有统计学意义(P<0.01).正常蛋白尿组血清 APN 为(3.27±0.68)mg /L ,低于健康对照组的(9.72±4.30)mg/L ,微量蛋白尿组、大量蛋白尿组血清脂联素水平分别为(10.88±2.85)mg/L 和(12.75±2.46)mg/L ,高于健康对照组,差异有统计学意义(P<0.01).结论血清 Cys C 、APN 可能参加了2型糖尿病肾病的发生和发展,检测其血清水平可能对早期诊断和评估 DN 有作用.

Correlation of changes of serum cystatin C and adiponectin(APN) level in patients with diabetic nephropathy

Objective To explore the roles of serum cystatin C(Cys C) and adiponectin in the pathogenesis and progress of nephropathy among type2 diabetic patients through serum levels detection.Methods A total of 105 patients with type 2 diabetes mellitus were divided into three groups according to the urinary albumin excretion rate(UAER):normal-albuminuria group(35 cases),micro-albuminuria group(35 cases) and macro-albuminuria(35 cases);with 35 healthy subjects included as controls.The mAlb,serum Cys C,APN,FBG,HbAlc,BUN and TG were observed in all subjects.Results Serum Cys C levels in three groups with diabetic patients were(1.72±0.57),(2.49±0.86),(3.97±1.20)mg/L respectively,these were significantly higher than controls(P<0.01).Serum adiponectin levels in the normal albuminuria group was(0.91±0.25)mg/L,it was over than controls,while these in the micro-albuminuria group and the macro-albuminuria group were(10.88±2.85),(12.75±2.46)mg/L,these were higher than controls(P<0.01).Conclusion Serum Cys C,adiponectin may play important roles in the occurrence of the diabetic and development of DN.Levels of serum cystatinc and adiponectin may help with the early diagnosis and evaluation of DN.
