消化道类癌的诊断和治疗

目的：探讨消化道癌经内镜诊治和手术治疗的适应证。方法；结合文献分析消化道类癌的临床表现，内镜下特点及治疗方法。结果；４３例消化道类癌均经内镜确诊，其中食管类癌２例，胃类癌８例，十二指肠球部类癌１例，结肠类癌３２例。我们分别采用内下电灼切除２６例，局部切除５例和根治手术１０例。２例术中发现已广泛转移。

DIGESTIVE TRACT CARCINOID:DIAGNOSIS AND TREATEMENT

Objective:To study the diagnosis and treatment by endoscope and the indication of surgical management in the patients with digestive tract carcinoid. Methods: We analyzed the clinical manifestation,character of the endoscope,treatment of 43 patients with digestive tract carcinoid cimbined with the literature.Result:43 cases of the digestive tract carcioid were diagnosed by endoscope,including 2 cases of esophagus carcinoid,8 cases of carcinoid,1 case of duodenal carcinoid,32 cases of colon carcinoid(31 rectum carcinoid,1 sigmoid colon carcinoid).26 cases were resected by electric cauterization under endoscope,5 cases were performed local resection,10 cases were performed radical operation.Widespread metastasis was discovered in 2 patients during the operation.Conclusion:The digestive tract carcinoid with less than 1cm diameter should be resected by electric cauterization under endoscope ,the focus ranging 1.1 2cm can be performed local resection,and the focus more than 2.1cm should be performed radical operation.
