食管癌切除胃代食管左颈部吻合162例临床分析

目的:探讨中上段食管癌切除,胃代食管经原食管床行左颈部吻合术的临床效果。方法:我院1995年3月～2001年12月采用此术式治疗162例,进行临床分析对比,总结该术式优点并提出手术操作注意问题。结果:全组切除151例,切除率达93.2％,术后并发症14例(8.6％),术后死亡率1.2％。临床效果较其他术式满意。结论:该术式具有操作方便、暴露好,比较符合原有的解剖及生理功能,减少术后心肺并发症及术后死亡率等优点。宜于在基层医院推广应用。

Analysis of 162 Cases of Esophagus - stomach Transition for Anastomosis through the Left Neck in Excising Esophageal Car-cinoma

Objective:To Probe into the effect of esophagus - stomach transition through the left neck and the original esophagus bed in treating patients with middle or upper part of esophageal cancer. Methods: From March 1995 to December 2001, 162 cases of esophagus cancer were treated by this operative procedure. This kind of operative procedure was compared with others and its advantages were summarized. It was also pointed out that some questions should be paid attention to when performing this kind of operation. Results: Esophageal cancer were excised in 151 patients(93.3%), the postoperative complications were 14(8.6%) and the death rate was 1.2%. The clinical result was better than that of other operation methods. Conclusion: This operation method has a lot of merits, for example, the operation is easier, the exposure is more extensive,and the anastomosis is in keeping with its original anatomical and physiological functions and may reduce the heart and lung complications. So this kind of operation is suitable for the basic hospitals of our country.
