肾盂输尿管连接部梗阻手术治疗的回顾性分析

目的总结肾盂输尿管连接部梗阻(UPJO)的诊治体会。方法回顾分析98例肾盂输尿管连接部梗阻患者的临床资料。其中肾盂输尿管连接部狭窄74例,肾盂输尿管高位连接11例,迷走血管压迫7例,纤维索压迫6例。行离断性肾盂成形术66例,非离断性肾盂成形术28例,肾切除术4例。结果98例随访6～48个月,非离断性肾盂成形术28例中,15例肾积水明显减轻(53.5%),8例无明显变化(28.5%),5例加重(17.8%);离断性肾盂成形术66例中,58例肾积水明显减轻(87.9%),6例无明显变化(9.1%),2例加重(3%)。结论离断性肾盂成形术是治疗肾盂输尿管连接部梗阻的首选术式。

Treatment of ureteropelvic junction obstruction with operation:An retropectively analysis

Objective To sum the experience on diagnosis and treatment of ureteropelvic junction obstruction(UPJO) with operation.Methods The clinical datas of 98 patients with UPJO were retrospectively analysed.Of these patients,74 patients with ureteopelvic junction constriction;11 patients had high ureteropelvic junction ;7 patients with compression from crossing blood vessels; 6 patients with compression from fibre cables.66 patients underwent dismembered pyeloplasty; 28 patients underwent undismembered pyeloplasty;4 patients underwent renal resection.Results 89 patients was followed up from 6 to 48 monthes.In these patients,28 patients had been subjected to undismembered pyeloplasty,among whom 15 patients resolved hydronephrosis,8 patients had no changes,5 patients became serious than before;while,among the 66 patients subjected to disembered pyeloplaty,58 patients' hydronephrosis were distincly resolved;in 6 patients there had no significant changes,2 patients became more serious than before operaton.Conclusion Dissembered pyeloplasty is the first choice for the patients with ureteropelvic junction obstruction.
