腹腔镜下子宫切除术三种不同手术方式效果比较

目的:分析腹腔镜下子宫切除术不同术式的临床疗效。方法:安阳市第六人民医院2008年1月～2011年8月行腹腔镜下子宫切除术的患者73例,回顾性分析腹腔镜全子宫切除术(TLH组)、腹腔镜次全子宫切除术(LSH组)和腹腔镜筋膜内子宫切除术(CISH组)手术及术后恢复情况。结果:手术时间、术中出血量,LSH组患者明显少于TLH组和CISH组(P<0.05);住院时间TLH组明显长于其他两组(P<0.05),LSH组明显短于其他两组(P<0.05);术后排气时间及下床时间比较,三组之间无统计学差异(P>0.05)。术后TLH组发生1例阴道出血,1例尿潴留,其他组均未发生并发症。结论:腹腔镜下三种不同的子宫切除术术式各有优势,临床选择何种术式重点要结合患者的具体情况,各种术式并不能完全互相替代。

Comparison of effects of three different operation methods of hysterectomy under laparoscope

Objective:To analyze the clinical efficacies of different operation methods of hysterectomy under laparoscope. Methods:Seventy-three patients who underwent hysterectomy under laparoscope in the hospital from January 2008 to August 2011 were selected,then the operation situations and recovery situations after operation in total laparoscopic hysterectomy group(TLH group),laparoscopic subtotal hysterectomy group(LSH group),classic intrafascial supracervical hysterectomy group(CISH group) were analyzed retrospectively. Results:The operation time and the amount of blood loss during operation in LSH group were significantly lower than those in TLH group and CISH group(P<0.05).The hospitalization time in TLH group was significantly longer than those in the other two groups(P<0.05).After operation,one case with vaginal bleeding and one case with urinary retention were found in TLH group,no complications occurred in the other two groups. Conclusion:The three different operation methods of hysterectomy under laparoscope have advantages and disadvantages,respectively,the specific circumstance of patients should be considered when to choose operation method,and one operation method can’t replace another completely.
