201例宫颈上皮内瘤变患者阴道镜检查结果分析

目的:为临床诊断宫颈上皮内瘤变(CIN)提供理论指导及统计分析依据。方法:对病理确诊的CIN患者的阴道镜资料进行统计分析,了解CIN级别与患者年龄、宫颈糜烂程度、阴道镜图像特征之间的相关性,并判断是否存在线性关系。结果:201例不同年龄阶段CIN病理类型构成比无统计学意义(P=0.090),随着年龄的增加,CIN病理类型呈现出线性趋势(P=0.016),并且年龄与病理类型相关,但相关程度较小(P<0.005)。不同级别CIN宫颈糜烂程度分布无统计学意义(P=0.994),随着CIN程度的增加,宫颈糜烂程度未呈现出线性趋势(P=0.660),并且CIN不同病理类型与宫颈糜烂程度不相关(P=0.675)。不同级别CIN图像特征构成比有统计学意义(P<0.001),随着CIN程度的增加,阴道镜图像特征呈现出线性趋势(P<0.001),并且图像特征与病理类型相关程度较大(P<0.001)。结论:①CIN级别与患者年龄、阴道镜图像特征之间存在线性关系,与宫颈糜烂程度无关。②阴道镜在CIN的诊断中具有重要价值,但操作者需注意提高阴道镜图像鉴识能力。

Analysis on results of colposcopy in 201 patients with cervical intraepithelial neoplasia

Objective:To provide theoretical guidance for clinical diagnosis of cervical intraepithelial neoplasia(CIN) and analyze the foundation statistically. Methods:The colposcopic data of CIN patients who were diagnosed definitely by pathological examination were analyzed statistically,the correlation between the grade of CIN and age,the degree of cervical ectopic columnar epithelium,and colposcopic image characteristics was understood,linear correlation was analyzed. Results:There was no statistically significant difference in the proportion of CIN pathological types among 201 patients in different age groups(P=0.090),the pathological types of CIN showed linear trend with age(P=0.016).There was a correlation between age and pathological types,but the degree of correlation was low(P<0.005).There was no statistically significant difference in the distribution of degree of cervical ectopic columnar epithelium among CIN of different degrees(P=0.994),the degree of CIN didn’t show linear trend with the increase of degree of CIN(P=0.660).There was no correlation between pathological types of CIN and the degree of cervical ectopic columnar epithelium(P=0.675).There was statistically significant difference in the proportion of image characteristics among CIN of different degrees(P<0.001),colposcopic image characteristics showed linear trend with the increase of degree of CIN(P<0.001),the degree of correlation between image characteristics and pathological types of CIN was high(P<0.001). Conclusion:There was linear correlation between the grade of CIN and age,colposcopic image characteristics,and there was no correlation between the grade of CIN and the degree of cervical ectopic columnar epithelium.Colposcope has important value for diagnosis of CIN,but the operators should improve the resolving ability of colposcopic images.
