血清Hcy与蛛网膜下腔出血后脑血管痉挛的相关性分析

摘    要:
目的 探讨蛛网膜下腔出血(SAH)患者血清同型半胱氨酸(Hcy)水平与脑血管痉挛(CVS)的相关性.方法 186例SAH患者中,将继发CVS的68例作为CVS组,未发生CVS的118例作为对照组,对比分析两组血清Hcy水平,并对Hcy水平与CVS的发生率进行相关性分析.结果 CVS组血清Hcy水平为(32.6±16.8)μmol/L,显著高于对照组的(16.2±17.8)μmol/L(t=6.272,P<0.05);血清Hcy水平>20 μmol/L者在CVS组中占47.1%,显著高于对照组的30.5%(χ^2=5.095,P<0.05).随着Hcy水平的升高,CVS的发生率逐渐升高,血清Hcy水平与CVS的发生率呈正相关(r=0.969,P<0.01).结论 血清Hcy水平与SAH后CVS相关,Hcy的检测有助于CVS的早期预测和干预.

Abstract:
[Abstract] Objective To explore the correlation between serum homocysteine (Hcy) levels and cerebral vasospasm (CVS) after subarachnoid hemorrhage (SAH). Methods Among 186 patients with SAH,68 ones with secondary CVS were regarded as CVS group,and 118 ones without CVS were considered as control group. The serum Hcy levels of the two groups were comparatively analyzed, and correlative analysis was carried out between Hcy levels and the incidence of CVS. Results The serum Hcy levels of CVS group [ (32.6 ±16.8) μmol/L ] were obviously higher than those in the control group [ ( 16.2 ±17.8 )μmol/L ] ( t = 6. 272, P 〈 0.05). The proportion of patients with serum Hcy level 〉 20 μmol/L in CVS group was 47.1%, which was significantly higher than that in the control group (30.5%) (Х^2 =5. 095 ,P 〈0.05). Along with the increase of the serum Hcy level,the incidence of CVS increased gradually. There was a positive correlation between the serum Hcy level and the incidence of CVS ( r = 0. 969, P 〈 0.01 ). Conclusion The Hcy level is correlative with CVS after SAH. The detection of Hcy is helpful to the early prediction and intervention of CVS.
