多层螺旋CT对胰岛细胞瘤的诊断价值

目的探讨多层螺旋CT在胰岛细胞瘤诊断价值。方法回顾性分析经手术病理证实的7例胰岛细胞瘤CT表现。其中功能性胰岛细胞瘤6例,无功能性胰岛细胞瘤1例。采用平扫和双期（动脉期和门静脉期）增强扫描,动脉期、门静脉期扫描延迟时间分别为25 s-30 s,60 s-70 s。结果功能性胰岛细胞瘤多较小,一般为1 cm-2 cm,而无功能性胰岛细胞瘤体积一般较大,大者可大于5 cm。7例胰岛细胞瘤均富血供,边界清楚,与周围胰腺组织分界清楚,未出现周围血管及胰胆管直接侵犯,较少出现腹膜后淋巴结转移。结论多层螺旋CT扫描对胰岛细胞瘤的检出、分期及判断预后有很大意义,可在术前提供重要信息。

Multi-slice spiral CT for the diagnosis of islet cell tumor

Objective：To evaluate MSCT scanning in the diagnosis of islet cell tumor. Methods：CT findings in 7 patients with surgically and pathologically proved islet cell tumor were retrospectively analyzed, which 6 are functional islet cell tumors and 1 nonfunctional. The CT findings were investigateld by using pre - contrast and contrast scanning （ including arterial and portal venous phase） The contrast - enhanced study started in 25 sand 70 s. Results ： The volume of insulinoms was relatively small, it is from one to two centimeter, while that of non - functional islet cell tumors were relatively large. It is 5 centimeter, greater. 7 pancreatic islet cell tumors were hypervascular with clear margin, no blood vessel and pancreatic duct infiltration . Islet cell tumors were less likely to invaded the common bile duct and pancreatic duct , and less likely to invaded peripancreatic vessels. Conclusion ： Multi - slice spiral CT, which can provide valuable information before surgery, plays an important role in the detection of tumors, staging, and determination of prognosis.
