磁共振血管造影及其原始图像、磁共振成像诊断脑动脉瘤

摘    要:
目的:回顾性分析了24例颅内动脉瘤病人的磁共振血管造影(MRA)及其原始图像,磁共振成像(MRI),数字减影血管造影(DSA)图像,评价其应用的价值和不足。方法:从各图像分析显示动脉瘤及其并发症信息的数量和质量,载瘤动脉、瘤颈、瘤腔的图像质量计分后统计处理。结果:检出率:DSA(95%)>MRA(87.5%)>MRI(68%),ERA 原始图像 MRI联合应用提高至92%,与DSA无明显差异;MRA有伪影,载瘤动脉细分支动脉和大瘤腔无信号;原始图像可发现小的和巨大动脉瘤,显示瘤腔、瘤壁、瘤体、占位效应准确,其瘤颈、瘤腔的信号多高于MRA;MRI显示瘤腔、瘤壁、瘤体、占位效应、出血、脑积水、脑水肿满意。结论:MRA及其原始图像联合应用相互验证同一信息,去除伪影,提高质量;信息全面,优于DSA;由于无创伤,无危险,对诊断动脉瘤有很高的实用价值。

Abstract:
To explore the value and the limitation of the MRA and its source images, MRI and DSA in the diagnosis of intracranial aneurysms. Method:24 patients with intracranial aneurysms was analysed retrospectively- The aneurysmal messages such as the parent artery, aneurysmal neck and lumen can be showed, then the quantity and quanlity of the images were scored, and the statistical analysis was made. Result: The detectable rate showed as follows: DSA (95 %) > MRA (87. 5 %) > MRI (68 %); The detectable rate of MRA source images MRI can increase to 92 % . There is no obvious difference in comparison with DSA. MRA had artifacts, the small arterial branch and giant aneurysmal sac can not be showed by MRA. The small or giant aneurysm, aneurysmal sac, wall, body, mass effect can be showed by the source images, its signals of aneurysmal neck and sac was higher than MRA. The aneurysmal sac, wall, body, mass effect, heamorrhage,hydrocephalus, brain edema can be showed satisfactorily by MRI. Conclusion: The combination of MRA and source images was used to verify the same informations, then the artifacts can be eliminated. The quanlity of images was improved, and superior to DSA, it has the high practical value for the diagnosis of cerebral aneurysm as a result of non invasive and without danger.
