Mirizzi综合征32例诊治体会

目的：总结Mirizzi综合征的诊断和治疗经验，探讨其临床特点。方法：对经手术证实的32例Mirizzi综合征的临床资料进行回顾性分析。结果：术前经ERCP及B超检查明确诊断者仅6例。行胆囊大部切除术21例，行胆囊切除加T管引流术5例，瘘口直接缝合修补3例，胆囊瓣瘘口修补2例，肝总管空肠吻合1例。结论：Mirizzi综合征术前诊断仍较困难，治疗应根据不同类型选择适当的手术方式。

Experience in diagnosis and treatment of 32 cases of Mirizzi syndrome

Objective To summarize the experience in diagnosis and treatment of Mirizzi syndrome and explore features of the disease. Methods Clinical data of 32 cases of surgically-proved Mirizzi syndrome were retrospectively analyzed. Results Only 6 cases were definitely diagnosed by ERCP and ultrasonography before operation. Of the 32 cases, 21 were treated with subtotal cholecystectomy, 5 with cholecystectomy and T-tube drainage of common bile duct, 3 with suture closure of the fistula, 2 with cholecystectomy and remnant of the gallbladder and 1 with Roux-en-Y hepaticojejunostomy. Conclusions It is still difficult to diagnose Mirizzi syndrome preoperatively. Appropriate operative patterns should be selected according to different types of Mirizzi syndrome.
