原位心脏移植后的超声心动图特征及演变

目的：报道原位心脏移植后超声心动图特征及演变的意义。方法：2例患者同种原位心脏移植术后定期彩色超声心动图检查，必要的心肌活检。结果：供体心房与受体心房吻合使左右心房呈“腰征”，心房上下径增大，房室瓣返流，返流可逐渐减轻或消失；术后4－5天出现心包积液，180天内消失，积液量与排斥反应有关；左室收缩功能与房室瓣返流程度及心包积液量无关。结论：原位移植心脏心房形态与自然心脏不同，定期超声心动图检查有重要临床意义。

Echocardiographic Characteristics and Transformation after Orthotopic Heart Transplantation

Objective: To report the significance of echocardiographic characteristics and transformation after orthotopic heart transplantation. Methods:Two patients who received homologous orthotopic heart transplantation were followed-up with echocardiography regularly and endomyocardial biopsies were done according to clinical necessity. Results: Donor-recipient atrial anastomosis protruded into both right and left atrial cavities and resulted in the "waist sign", the up-down dimension of the atria is lengthened. There was atrioventricular valvular regurgitation, the severity of which could be decreased or disappeared time-de-pendently. Pericardial effusion appeared within 4～5 days after the operation and disappeared within 180 days. The quantity of pericardial effusion is associated with acute rejection. Atrioventricular valvular regurgitation and the quantity of pericardial effusion had no significant relationship with the left venticle systolic function. Conclusions:The shape of the orthotopic heart transplant is obviously different from that of the natural heart. Echocardiography is useful in the follow-up of heart transplant recipients
