中医情志护理配合针刺治疗脑卒中后抑郁临床研究

目的:观察中医情志护理配合针刺治疗脑卒中后抑郁的临床效果。方法:选取90例脑卒中后抑郁患者,按随机数字表法分为观察组和对照组各45例,对照组给予针刺与常规护理,观察组在对照组基础上给予中医情志护理。观察比较2组临床疗效,评价汉密尔顿抑郁量表(HAMD)评分和Barthel指数,检测血清5-羟色胺(5-HT)水平。结果:观察组总有效率为91.11%,显著高于对照组64.44%(P<0.05)。干预前,2组HAMD评分比较,差异无统计学意义(P>0.05)。干预后2周、4周,2组HAMD评分较干预前明显降低(P<0.05),且观察组HAMD评分低于同期对照组(P<0.05)。干预前,2组血清5-HT水平及BI指数评分比较,差异无统计学意义(P>0.05)。干预后,2组血清5-HT水平较干预前明显上升(P<0.05);2组BI指数评分较干预前明显升高(P<0.05),且观察组BI指数评分显著高于对照组(P<0.05)。结论:中医情志护理配合针刺治疗脑卒中后抑郁可有效提升临床治疗效果,改善患者抑郁症状,提高血清5-HT含量及日常生活自理能力,值得临床推广实施。

Clinical Study on Emotional Nursing of Chinese Medicine Combined with Acupuncture for Post-stroke Depression

Objective:To observe the clinical effect of emotional nursing of Chinese medicine combined with acupuncture for post-stroke depression.Methods:A total of 90 cases of patients with post-stroke depression were selected and divided into the observation group and the control group according to the random number table method,45 cases in each group.The control group was given acupuncture and routine nursing,and the observation group was additionally given emotional nursing of Chinese medicine based on the treatment of the control group.The clinical effect in the two groups was observed and compared,the scores of Hamilton Depression Scale(HAMD)and Barthel indexes were evaluated,and the levels of 5-hydroxytryptamine(5-HT)in serum were detected.Results:The total effective rate was 91.11%in the observation group,significantly higher than that of 64.44%in the control group(P<0.05).Before intervention,compared the HAMD scores in the two groups,there was no significance in the difference(P>0.05).In two and four weeks after intervention,HAMD scores in the two groups were significantly decreased when compared with those before intervention(P<0.05),and HAMD score in the observation group was lower than that in the control group(P<0.05).Before intervention,compared the levels of 5-HT in serum and the scores of Barthel indexes in the two groups,there was no significance in the difference(P>0.05).After intervention,the levels of 5-HT in serum in the two groups were significantly increased when compared with those before intervention(P<0.05);the scores of Barthel indexes in the two groups were significantly increased when compared with those before intervention(P<0.05),and the above scores in the observation group were significantly higher than those in the control group(P<0.05).Conclusion:The therapy of emotional nursing of Chinese medicine combined with acupuncture for post-stroke depression can effectively improve the clinical effect and the depressive symptoms of patients,and enhance the content of 5-HT in serum and self-care ability,which is worthy of clinical promotion and application.
