高迁移率族蛋白B1在急性主动脉夹层中的表达及意义

摘    要:
目的:分析高迁移率族蛋白B1(HMGB1)与急性主动脉夹层(AAD)的关系,探讨HMGB1作为AAD生物学标志的意义。方法:选择2009-01-2013-05在我院确诊为AAD的患者46例和正常对照患者37例,常规测量血压、生化指标、超敏C反应蛋白(hsCRP)。采用ELISA法检测血清HMGB1水平。对于进行主动脉置换术或主动脉内支架植入术的患者以及死亡的患者,检测出院前或死亡前的血清HMGB1水平。结果:AAD组和对照组之间的年龄、血压、肝肾功能、心肌酶、血糖、血脂差异无统计学意义(P〉0.05)。AAD组患者的hsCRP和血清HMGB1水平明显升高(P〈0.05);进行主动脉瓣膜置换或主动脉内支架植入治疗后,HMGB1水平显著下降(P〈0.05),而死亡的AAD患者的血清HMGB1水平维持在较高的水平,与入院时的水平相比无明显下降(P〉0.05)。结论:血清HMGB1可能参与了AAD的发生发展,在AAD的血清诊断和预后判断中具有一定意义。

Abstract:
Objective:To investigate the relationship of high mobility group protein B1(HMGB1)with acute aortic dissection(AAD)and to evaluate the significance of HMGB1serving as biomarker of AAD.Method:46cases of AAD patients and 37cases of normal controls were enrolled from January 2009to May 2013.Blood pressure,biochemical variants,high sensitivity C-reactive protein(hsCRP)were routinely identified.The concentrations of serum HMGB1were measured by ELISA.Those patients who underwent surgery operation or aortic intervention or died were arranged for serum HMGB1measurement before they discharged or died.Result:There were no significant differences in age,blood pressure,liver and kidney function,myocardial enzyme,blood glucose,blood lipids between AAD group and control group(P〉0.05).The serum levels of HMGB1were significantly decreased after aortic valve replacement or aortic stent implantation(P〈0.05).However,the serum HMGB1of dead patients did not decrease from admission to death(P〈0.05).Conclusion:HMGB1may be involved in the development of AAD and could contribute to the diagnosis and prognosis evaluation of AAD.
