学龄期癫痫患儿治疗前后认知功能状况及其相关因素的研究

目的探讨学龄期癫痫患儿治疗前后认知功能的改善情况，并分析其相关因素。方法回顾性分析来我院就诊的学龄期癫痫患儿115例，采用韦氏儿童智力量表中国修订版调查患儿治疗前后的言语智商、操作智商及总智商，比较治疗前后的差异。结果治疗后患儿的言语智商、操作智商及总智商均较治疗前有显著的改善（P〈0．05），不同的治疗方法、不同的发病年龄、不同的病程、每年癫痫发作不同次数及每次发作持续不同时间间总智商比较，差异均有统计学意义（P〈0．05），而患病到治疗时间不同，不同的发作类型各组间比较，差异无统计学意义。结论学龄期癫痫患儿认知功能障碍与药物的治疗方法、发病年龄、病程、每年癫痫发作次数及每次发作持续时间有关。在治疗时应综合患者各方面的因素，尽量避免影响其认知功能下降的因素，提高临床治疗效果。

Study of the Correlative Factors and Cognitive Function of School Age Children with Epilepsy Before and After Treatment

Objective To study the correlative factors and cognitive function of school age children with epilepsy before and after treatment. Methods Retrospective analysis of 115 school age children with epilepsy in our hospital, verbal IQ （ VIQ ）, performances IQ （ PIQ ） and the full intelligence quotient （ FIQ ） were surveyed using WISC-CR revised edition in China. We compared the differences between before and after treatment. Results After treatment, VIQ, PIQ and FIQ were improved compared with before treatment （ P 〈 0.05 ）. FIQ has a significant difference between different treatment, different age at oneset, different different course of disease, different frequency of epilepsy, different in seizure-lasting time （P 〈 0.05 ）. But different time from diabetes to treatment and different epilepsy types had no significant difference. Conclusion The cognitive impairment is correlated with treatment of choice, age at oneset, course of disease, frequency of epilepsy and seizure-lasting time. We should synthesize various factors of patients and try to avoid risk factors of cognitive decline, to impove the clinical result.
