微创经皮肾穿刺取石术治疗上尿路结石的临床研究

目的:对微创经皮肾穿刺取石术(Mini-PCNL)治疗尿路结石的临床效果进行观察。方法:对2008年12月～2010年12月入住泌尿科的80例尿路结石患者的临床资料进行回顾性分析,将本组患者随机性地分为对照组与观察组,每组各40人,观察组采用微创经皮肾穿刺取石手术,对照组采用常规的取石术。对比分析2组在碎石成功率以及结石排净等情况。结果:观察组一次性结石清除人数为39人(97.5%),患者碎石率为100%,对照组上述两个比率则要明显地低于观察组(分别为77.5%和70.0%)(P<0.01)。住院时间2组差异无统计学意义(P>0.05)。患者并发症发生率,观察组明显小于对照组(P<0.01)。结论:微创经皮肾穿刺取石术对患者尿路结石治疗具有创伤小、一次性排石率高、手术成功率高且并发症少,为治疗上尿路结石的一个非常有效的微创手术方法,值得广泛推广。

Effect of minimally invasive percutaneous nephrolithotomy on upper urinary calculi

Objective: To explore effect of minimally invasive percutaneous nephrolithotomy on upper urinary calculi.Methods:Clinical data of 80 cases with upper urinary calculi admitted from December 2008 to December 2010 were analyzed retrospectively.All patients were randomly divided into control group and observation group by half.Patients in observation group were treated by minimally invasive percutaneous nephrolithotomy;while patients in control group were treated by conventional lithotomy.The success rate and removal status were observed and compared.Results: A total of 39 patients had calculi cleared(97.5%),and the stone-free rate was 100%.In observation group,which were significantly higher than those of control group(P<0.01).There was no significant difference in length of stay(P>0.05).And the incidence of complication was significantly lower in observation group(P<0.01).Conclusions:Minimally invasive percutaneous nephrolithotomy is of minimal incision,high success rate and low incidence of complication.It is effective on treating upper urinary calculi,and is worthy clinical application.
