摘    要:目的:探讨小儿急性呼吸窘迫综合征在治疗时机械损伤的预防。方法:以2009年1月~2011年1月期间本院收治的68例小儿急性呼吸窘迫综合征患者为研究对象,随机分为观察组和对照组,对照组给予常规机械通气方法,观察组给予肺保护性机械通气策略,观察两组患者在呼吸机参数设置、并发症发生率、死亡率、呼吸机使用时间方面的差异。结果:观察组呼吸机参数显著优于对照组,呼吸机相关性肺损伤和脑室内出血的发生率低于对照组,呼吸机应用天数与对照组比较无统计学意义。结论:肺保护性通气策略能够指导急性呼吸窘迫综合征患儿的呼吸机治疗,降低机械通气相关性肺损伤的发生率。

Abstract:Objective:To explore the prevention of mechanical injury in children with acute respiratory distress syndrome.Methods:A total of 68 cases of acute respiratory distress syndrome children that admitted to our hospital during January 2009 and January 2011 were divided randomly into trial group and control group,the control group was given conventional mechanical ventilation while the observation group was given lung protective mechanical ventilation strategy.The set machine parameters,complications,mortality,and length of mechanical ventilation consuming time were observed and compared between the two groups.Results:The observation group showed significant better machine parameters,and lower incidence of mechanical injury and intraventricular hemorrhage,but no significant difference in the length of mechanical ventilation consuming time was found.Conclusions:Lung protective mechanical ventilation strategy can be reference method for treating children with acute respiratory distress syndrome since it can reduce the incidence of the mechanical ventilation correlated lung injury.
