桡骨远端不稳定骨折两种治疗方法效果比较

目的:比较锁定加压钢板内固定与手法复位闭合外固定治疗桡骨远端不稳定骨折效果。方法:采用随机对照设计将60名桡骨远端不稳定骨折患者随机分配到锁定加压钢板内固定组(30例)与手法复位闭合外固定组(30例)进行治疗,随访1年后比较两组患者Mcbride评分。结果:两组患者性别、年龄、骨折分型差异无统计学意义,具可比性。随访1年后,钢板内固定组优良率为83.3%,闭合外固定组优良率为60.0%,钢板内固定组Mcbride评分优于闭合外固定组(Z=3.307,P=0.001)。结论:锁定加压钢板内固定与传统手法复位闭合外固定相比,能够有效的稳定骨折复位,使患肢及早进行功能锻炼和恢复。

Comparison of two treatments of unstable distal radius fractures

Objective: To compare the effect of internal fixation with locking compression plate and closed external fixation with manual reduction on unstable distal radius fractures.Methods:A total of 60 patients with unstable distal radius fractures were randomly allocated into internal fixation with locking compression plate group(30 patients) and closed external fixation with manual reduction group(30 patients).The Mcbride score of the two groups was compared after one year follow-up.Results:There were no significant differences in sex,age,and classification of fracture between the two groups,indicating a comparable baseline.After one year follow-up,a significant difference of Mcbride was observed between the two groups(Z=3.307,P=0.001).The effect of internal fixation with locking compression plate was superior to closed external fixation with manual reduction,and the excellent rate were 83.3% and 60.0%,respectively.Conclusions:The internal fixation with locking compression plate performs excellent in treating unstable distal radius fractures.Compared with traditional closed external fixation with manual reduction,it stabilizes fractures effectively so that patients can begin the restorative training early.
