上海市松江区社区自然人群大肠癌筛查结果分析

目的了解大肠癌及其相关因素的流行情况，探讨自然人群大肠癌筛查的效果。方法应用卫生部《癌症早诊早治项目技术方案（2011年版）》中推荐的方法，对上海市松江区40～79岁户籍自然人群开展大肠癌筛查，并对结果进行分析。结果收回14563份有效数据，初筛阳性率为52．0％，精筛阳性率为31．9％；问卷组的肠镜顺应性、癌及癌前病变检出率均低于FOB组；男性肠镜顺应性、患病率均高于女性。结论（1）问卷结合FOB试验可以有效地检出大肠癌及癌前病变；（2）工作忙是多数中年居民无法进行筛查的主要原因；（3）社区居民初筛阳性率高、精筛依从性低；（4）男性患病率明显高于女性；（5）与问卷调查相比，FOB试验还不被居民看重，需加强宣传力度。

Analysis on the Result of Colorectal Cancer Screening on Natural Population in Community of Songhua District in Shanghai

OBJECTIVE To investigate the prevalence of the colorectal cancer and its related factors, to explore the effects of the colorectal cancer screening on natural population. METHODS We screened natural population with age from 40 to 79 in ShangHai SongJiang Community, using the screening program recommended by the ministry of health for Cancer Early Detection and Treatment, then analysised the results. RESULTS We took back 14 563 valid data, the positive rates of preliminary screening and final screen- ing were 5：2.0% and 31.9%. The compliance rate of eolonoseopy and detection rate of cancer in questionnaire group were both lower than in FOB group. Male＇s compliance rate of colonoscopy and prevalence rate were both higher than Females＇. CONCLUSION Questionnaire accompanied by FOB test is efficient in detecting colorectal cancers.The main reason of most middle-aged people missing screen is their busy work. Community people＇ s positive rates of preliminary screening is high while the compliance rate of final screening was low. Male＇ s prevalence rate is significantly higher than Female.The questionnaire is less acceptable by population than the FOB test, we have to increase the propaganda.
