摘    要:应用放射免疫法对6例急性肾功能衰竭(ARF)患者尿中表皮生长因子(EGF)的水平进行测定和动态观察,并用免疫组化法对其中5例患者肾活检组织中肾小管上EGF和EGF受体(EGF-R)进行定量分析。结果ARF恢复期尿中EGF含量的不断增加,血清肌酐水平和钠滤过分数逐渐下降,在伴有肾小管上皮细胞再生表现者其肾小管上EGF的增加尤为明显,并且与肾活检时尿中EGF含量的变化相平行。揭示在ARF恢复期尿中EGF含量的变化可反映肾小管和肾功能的恢复状态,而肾小管上EGF的增加可能有利于肾小管上皮细胞的再生。

Abstract:Epidermal growth factor (EGF) plays an important role in modulating mitogenic and metabolic activity of renal tubular epithelial cell. In this study, serial observation of EGF in the urine and EGF, EGF recep tor (EGF-R) in the renal tissue were made in 6 cases of acute renal failure by RIA or immune peroxidase technique (4 layer PAP method). It was found that urinary EGF concentration (ug/L) increased progressively toward the convalescent period of ARF accompanied with the declining serum creatinine level and fraction of excretion of fil tered sodium. Renal biopsies were performed during convalsecence of ARF- It was found that specimens showed prominent tubular regeneration tending to have abundant amount of EGF in renal tubules being paralled with the increment of EGF excretion in urine. It was suspected that sequential detection of EGF in urine might serve as an index to reflect the situation of renal tubular repairment and the state of renal function. The increment of EGF in renal tubules would indicate that EGF may play an important role in the regeneration process of renal tubular epithelium during human ARF.
