老年重症肺炎患者早期乳酸清除率与预后的临床分析

目的探讨早期血乳酸清除率评估老年重症肺炎患者与预后的关系和意义。方法对62例老年重症肺炎入科时动脉血乳酸水平与治疗6h血乳酸清除率及患者的预后情况进行分析，并将患者分成存活组和死亡组、高乳酸清除率组和低乳酸清除率组，比较其上述指标的差异性。结果存活组、死亡组患者治疗前动脉血乳酸水平分别为（5．20±1．56）mmol／L及（5．60±1．40）mmol/L，比较差异均无统计学意义（均为P〉0．05）；存活组6h乳酸清除率明显高于死亡组（P〈0．01）；高乳酸清除率组和低乳酸清除率组患者治疗前动脉血乳酸水平分别为（5．30±1．54）mmol／L及（5．70±1．39）mmol／L，比较差异均无统计学意义（均为P〉0．05）。高乳酸清除率组与低乳酸清除率组的呼吸衰竭发生率、休克发生率、死亡率比较，均有显著性差异（P〈0．01）。结论早期乳酸清除率可用于早期评估老年重症肺炎的预后，并指导进一步治疗。

Prognosis of Elderly Severe Pneumonia Patients Assessed with Early Lactate Clearance Rate
