把握“特应性”涵义及其在皮炎湿疹诊断中的意义

明确特应性皮炎(atopic dermatisis,AD)的定义和范畴,区别特应性皮炎与其他皮炎/湿疹,关键在于对"特应性"的理解。变应性证据和基因研究,提示过敏和遗传是特应性形成的重要基础,但并非是唯一关键因素。一些非变应性因素如β-肾上腺素能受体反应减低、T细胞功能异常等均成为特应性形成的重要机制之一。因此,把握"特应性"涵义,在疾病诊断、内源性AD的认识以及合理治疗上均有指导意义。

The Concept of Atopy: Correct Definition and Its Role in the Diagnosis of Dermatitis

It is a key point to understand the concept of atopy correctly in order to clearly define the definition and category of atopic deramtitis(AD) and to differentiate the atopic and non atopic dermatitis.It has been shown that neither the allergy or the genetics are only key pathogenic factors although there is a strong link between AD and the factors.Some non-allergic factors,such as decreased β-adrenergic receptor responsiveness and impaired T cell function,etc.,sometimes play an important role in the pathogenesis of AD.Therefore,to identify the concept of atopy is significant for the diagnosis of AD,recognition of intrinsic type AD and rational application of various therapies.
