Sox2和Cdx2基因编码蛋白在胃癌及其癌前病变中的表达及意义

目的:采用免疫组化法观察研究Sox2和Cdx2基因编码蛋白在胃癌及其癌前病变组织中的表达并分析其与胃癌临床病理生物学行为的关系，探讨Sox2、Cdx2异常表达在胃癌发生发展中的作用机制及相互关系。方法:收集中国医科大学附属第一医院肿瘤外科手术切除胃癌标本257例，采用Envision免疫组织化学染色方法和HID-ABpH2.5-PAS黏蛋白组织化学染色法,观察Sox2和Cdx2在胃癌及其癌前病变中的表达，分析其表达的相互关系。结果:胃癌组织中Sox2的阳性表达率（64.98%）显著低于正常胃黏膜（89.02%）、肠上皮化生（76.38%）和异型增生（71.01%）中的阳性表达率,P均<0.05;弥漫型胃癌（71.54%）中的表达显著高于肠型胃癌（58.25%）,P<0.05。Cdx2蛋白在胃癌组织中的阳性表达率（53.93%）明显高于正常胃黏膜中的阳性表达率(6.74%),在肠上皮化生组织中的阳性表达率（88.89%）显著高于胃癌组织的阳性表达率，肠型胃癌中(68.75%)的表达水平高于弥漫型胃癌(40.38%）,P均<0.05。结论:Sox2对胃表型的维持起重要作用，可能与弥漫型胃癌的发生关系更为密切。Cdx2对肠表型的维持起重要作用，可能与肠型胃癌的发生更为密切。

The significance and expression of Sox2 and Cdx2 protein in gastric carcinoma and pre-cancerous lesions

Objective:To investigate the significance and expression of Sox2 and Cdx2 protein in gastric carcino-genesis and precancerous lesion,immunohistochemical stainning was used to detect the expression of Sox2 and Cdx2 protein in gastric carcinogenesis and precancerous lesion. Authors analyzed the expression of Sox2 and Cdx2,com-pared with their clinicopathological features of gastric carcinoma,and explored their roles in progression of gastric car-cinoma and relevant molecular pathological mechanisms. Methods:The material studied was taken from the oncosur-gery,No. 1 Hospital of China Medical University,consisted of 257 surgically resected specimens from patients with gastric carcinoma. Envision immunohistochemistry and double staining and HID - ABpH2. 5 - PAS mucin histochem-istry were used to observe the expression of Sox2 and Cdx2 in gastric carcinoma and precancerous lesions. The rela-tionship with their expression was analyzed as well. Results:The expression of Sox2 was the highest in the normal epi-thelium(89. 02% ). Sox2 in intestinal - type carcinoma(58. 25% )was less expressed than that in diffuse - type carcinoma(71. 54% ),P < 0. 05. Cdx2 in intestinal carcinoma(68. 75% )was expressed more than that in diffuse carcinoma(40. 38% ),P < 0. 05. Conclusion:Sox2 plays an important role in the maintenance of gastric phenotype and may be closely associated with diffuse - type gastric carcinoma. Cdx2 plays an important role in the maintenance of intestinal phenotype and may have a close correlation with intestinal - type carcinoma.
