MSCT诊断各级别阑尾炎的差异及与病理结果的一致性

目的 探讨MSCT诊断各级别急性阑尾炎的差异及与病理结果的一致性。方法 回顾性分析本院临床疑诊急性阑尾炎并手术的患者166例,对所有患者均进行常规MSCT扫描及MPR,92例患者加做CPR,根据影像征象分为0~5级,分别计算MSCT诊断各级别急性阑尾炎的准确率,并计算总体敏感度、特异度、准确率及与病理诊断的一致性。结果 MSCT诊断0~5级急性阑尾炎的准确率分别为93.33%(14/15)、54.55%(6/11)、89.47%(34/38)、95.52%(64/67)、91.30%(21/23)、91.67%(11/12)。MSCT诊断急性阑尾炎的敏感度为96.68%(146/151),特异度为93.33%(14/15),准确率为96.38%(160/166);影像诊断与病理诊断具有较高的一致性(Kappa=0.82)。结论 MSCT诊断急性阑尾炎与病理有较高一致性,其中对3级阑尾炎诊断准确率最高。

Difference of MSCT diagnosis of appendicitis at all levels and consistency with pathology

Objective To investigate the difference of MSCT in diagnosis of appendicitis levels and the consistency with the pathological results. Methods Totally 166 patients with clinical suspected diagnosis of appendicitis were enrolled. All patients underwent conventional MSCT and MPR, 92 cases were performed CPR. According to the imaging signs, the appendicitis was divided into level 0 to 5, and accuracy was calculated at all levels. The overall sensitivity, specificity, accuracy and consistency with pathology were also calculated. Results Diagnostic accuracy of 0 to 5 level appendicitis in MSCT were 93.33% (14/15), 54.55% (6/11), 89.47% (34/38), 95.52% (64/67), 91.30% (21/23), 91.67% (11/12). Overall sensitivity, specificity, accuracy were 96.68% (146/151), 93.33% (14/15), 96.38% (160/166). The image diagnosis and pathology had high consistency (Kappa=0.82). Conclusion MSCT in the diagnosis of appendicitis has high consistency with pathology, among them, the accuracy of diagnosis level 3 appendicitis is the highest.
